Pressure Ulcer Prevalence

Spaulding Cape Cod’s goal for hospital acquired pressure ulcers is zero. To reach that goal, SCC has an active interdisciplinary Wound Care Committee involving Physicians, Certified Wound Care Nurses, a Dietitian, and Physical and Occupational Therapists. The team not only evaluates and treats patients who have pressure injuries, but works with all patients and families to prevent pressure injuries from occurring.

Falls

Our comprehensive team approach to care lends itself to preventing falls. Our patients are assessed upon admission and throughout their stay regarding potential for falls and safety awareness. Fall prevention education to patients and families is provided throughout the patient’s stay. While our goal is to reduce our patient falls to zero, SCC balances increasing patient independence during their rehabilitation with fall prevention measures. If a fall does occur, our interdisciplinary team immediately reassesses the patient’s plan of care, making changes where necessary. In using this multifaceted approach, patient falls have remained well below national benchmarks.
